PRACTICE CONSIDERATION

USE THE FOLLOWING ACTIVITIES FOR EACH TIME PERIOD ASSIGNED BY YOUR INSTRUCTOR

DETERMINE WHICH ACTIVITIES ARE NEEDED

After reviewing the information in the Electronic Health Record (if provided), your "inbox", and Story Information, information regarding the timeframes for various MDS and CAA forms and compare data to criteria for Significant change in status (Care Area Triggers). NOTE: You will want to calculate the percentage of weight change at the various time periods (30 day, 60 day, 90 day and 180 day) prior to deciding which activities are needed.

Save the email you received with your answers to these questions as a pdf. After you have completed all activities upload this as the first document when you submit your assignments for a specific time period (30 day, 60 day .....12 Months)
