15.11 Residential Care-Skilled Nursing - Select Activities
Part of the clinical judgement of the dietitian in a Residential Care or Skilled Nursing Facility is the determination of which activities and forms need to be completed. While each organization sets their own requirements, most want to be eligible to receive Medicare/Medicaid reimbursement and therefore comply to the CMS regulations as well as State and local policies/procedures.
Be sure to review the Perspective and the Initial Interview background information on how this particular facility operates. Review the MDS timeline and the institutional policy and procedure exerpts. The links to these resources are on the Activities and Resource page.
After you have reviewed the electronic health record for the time period, interview videos, and documents in your "inbox" then complete this form to document your thinking on which activities you need to complete.
Based on the current data, comparing current data to previous data and your knowledge of the requirements to meet CMS and your institutional requirements, review the list of forms and actions below and indicate which activities you need to complete today. In the right hand column check the reason you selected the activity, e.g. indicate CMS requirement, change in patient nutritional status, or Institutional policy/requirement.
NOTE: The answers to this form will be the activities that you then complete in the next activity.
