The Nutrition Care Process (NCP) is a systematic, science based approach to providing nutrition care that also takes account of the patient’s needs and values.   Using the NCP model, clinicians have developed more than 1000 Nutrition Assessment-Monitoring and EvaluationTerms, 100 nutrition diagnoses, more than 300 terms to describe nutrition interventions, and in excess of 170 nutrition indicators for assessing, monitoring and evaluating nutrition status of individuals or groups of individuals.

The style of this resource and the information it contains are unique. Using a case-study approach, we offer students, practitioners and clinicians the opportunity to analyze the circumstances of the case as described by the patient and then draw on the evidence base to treat the condition. In the course of that process, we introduce the new dietetic tools and concepts needed to treat the case, showing how they take effect.

Coronary Heart Disease-Telehealth-Interpreter/Case 14

Case includes nutrition care to be provided to Jorge after discharge from recent coronary artery bypass surgery.  Jorge's appontment will be held via telehealth and needs an interpretor. Most of the information is included in the Electronic Health Record and student will need to extract pertinent information.  Seventeen activities includes preparation for using an interpretor and guides the student through the consultation using the Nutrition Care Process steps and activities. 

Residential Care-Skilled Nursing Rotation/Case 15

Case includes nutrition care to be provided to Bertha from admission to 12 months in a residential care facility.  Students completed the required CMS evaluation and documentation for admission and each month afterward.  Case includes experience using an electronic health record, changes in texture modifications, weight gain, weight loss, and entering into hospice care.   If students complete the entire 12 months of care it will include up to 67 separate activities (e.g. completing CAA 12, Completing MDS, Section K, Completing monthly risk tracking, Local institutional care coordination forms. and attending Care Planning Meetings.)
